Your In-Home Assessment

A coordinator visits your loved one at home to get a full picture — their health needs, daily habits, personal preferences, and the layout of their living space. This is how we build a care plan that actually fits your family’s life, not a generic checklist. We also walk through Medicaid eligibility at this stage if applicable.

Handpicked for Your Loved One

Your Caregiver Match

We handpick a caregiver based on far more than availability. We consider personality, experience with specific conditions like Alzheimer’s, dementia, or Parkinson’s, language preference, and schedule compatibility. If the match does not feel right for any reason, we find a better one — no awkward conversations needed.

Continuous Coordination

In most cases, we can begin care within 24 to 48 hours of your initial call. We respond the same day you reach out and schedule an in-home assessment within 48 hours. For urgent situations — such as after a hospital discharge — we do our best to fast-track the process.
