How to Choose a Home Caregiver

Choosing a caregiver for someone you love is one of the hardest decisions a family can make. You want someone who is skilled, trustworthy, and genuinely caring — not just a name on a list. The good news is that when you know what to look for, the process becomes much more manageable.

This guide walks you through every step: what to assess before you start, what to ask during interviews, what to watch for during a trial period, and what red flags should make you pause. Work through the interactive checklist items as you go.

Step One

Know What Kind of Care You Need

Before you start searching, take time to write down exactly what help your loved one needs day to day. This isn’t just about tasks like bathing or cooking — it’s about the whole picture: their medical needs, their personality, their daily routine, and what kind of person they’ll be comfortable with.

There are two broad categories of home care:

Step Two

Decide: Agency or Independent Caregiver?

This is one of the most important early decisions you’ll make. Both options have real advantages — it depends on your situation, your budget, and how much you want to manage on your own.

A licensed home care agency like CareWorks Houston handles hiring, background checks, training, scheduling, and payroll. If a caregiver calls in sick, the agency provides a backup. Agencies are also accountable to state regulations and licensing requirements.

An independent (private-hire) caregiver is someone you hire directly — often found through referrals, online platforms, or community networks. The cost is usually lower, but you take on the legal and administrative responsibilities: taxes, insurance, backup coverage, and vetting.

Something to consider: If your loved one has complex needs, memory issues, or requires consistent care, an agency typically provides more stability. Independent caregivers can work well for families who are highly involved and prefer a longer-term direct relationship.

Step Three

Screen Candidates Before the Interview

Before you spend time on an interview, do a basic screening to make sure the candidate meets your minimum requirements. This can be done by phone in 10–15 minutes and will save you a lot of time.

Red flag: A caregiver who is reluctant to authorize a background check or who avoids clear answers about their experience should not move forward in your process, regardless of how well the conversation goes otherwise.

Step Four

Ask the Right Interview Questions

The interview is your chance to go deeper. You want to understand not just their skills, but their approach to caregiving — how they handle difficult situations, how they communicate, and how they treat the people in their care.

If possible, include your loved one in the interview, or at least introduce them briefly. The emotional comfort between a caregiver and the person receiving care is just as important as qualifications.

Here are questions worth asking:

“Tell me about the most challenging caregiving situation you’ve faced and how you handled it.”
“How do you handle it when the person you’re caring for refuses help or becomes upset?”
“Are you comfortable with [specific task — e.g., memory care, transfers, wound care]?”
“What does a good day of caregiving look like to you?”
“How do you prefer to communicate with family members? How often?”
“Have you ever worked with someone who has [your loved one’s diagnosis]?”
“What would you do if you noticed a change in their health or behavior?”
“Why do you do this work? What keeps you in caregiving?”

Pay attention to how they answer — not just what they say. Do they speak about their clients with warmth and dignity? Do they seem patient? Are they comfortable with silence? These qualities matter as much as certifications.

Step Five

Check References and Run a Background Verification

Never skip references. Ask for at least two previous employers or families they’ve worked with directly — not just character references from friends. When you call, keep the conversation relaxed but be specific.

Good questions to ask references:

  • How long did they care for your family member?
  • Were they reliable and on time?
  • How did your loved one respond to them?
  • Did anything ever concern you about their approach?
  • Would you hire them again?

Background checks should include criminal history, sex offender registry, and — if driving is required — a motor vehicle record check. In Texas, home care agencies are required by the Texas Health and Human Services Commission (HHSC) to conduct these checks for all employees.

Step Six

Do a Trial Period — and Trust What You Observe

Even after a great interview, plan for a supervised trial period of one to two weeks before committing long-term. During this time, try to be present for at least part of each shift. This is especially important if your loved one has memory issues, limited mobility, or difficulty communicating their own needs.

Trust your instincts and your loved one’s reactions. If the person receiving care becomes withdrawn, anxious, or expresses discomfort — even if they can’t explain why — take it seriously. Your loved one’s comfort and safety come first, always.

Step Seven

Set Up a Care Plan From Day One

Once you’ve chosen a caregiver, document everything. A written care plan protects everyone — your loved one, the caregiver, and your family. It removes guesswork and gives the caregiver a clear guide to follow, especially in emergencies.
